When HIPAA was created, researchers studied the patterns of psychotherapists and found that most did a 45-minute session, leaving 15 minutes for notes, or they did a 60-minute session with notes being done at a later time. So, CPT codes 90834 and 90837 were created. For insurance reimbursement, it's dumbfounding that we must medically prove why we need the extra 15 minutes for a 60-minute session. The insurance rate for a 60-minute session doesn't even cover the regular going rate. And yet, we have to prove from a medical standpoint why an extra 15 minutes is needed when this 15-minute code wasn't even created from a medical standpoint. That's so fucking crazy. It's all a game.
